At Divine Care Support Services, we prioritise both the wellbeing of our participants as well as the wellbeing of primary carers who occasionally need a break. Our experienced support workers will come to your home and take care of your loved ones whilst you rejuvenate.

Post-Hospital Care Package Includes
At Divine Care Support Services, we offer transition care in the comfort of your home that helps you recover after a hospital stay. We provide short-term specialised care tailored to your needs and goals to help you regain your functional independence and confidence thereby avoiding the need for longer term care and support services.
- Community Nursing
- Assistance with household tasks (cleaning and cooking)
- School drop-off and pick
- Transportation to appointments, shopping and social activities
- Running errands and shopping with clients or on their behalf
- Assistance with collecting mail and paying bills
- Garden maintenance
- Welfare checks – either by phone or home visits
- Assistance with medication management
- Liaising with other relevant service providers
- Assistance personal care
- Assistance with mobilisation activities, including positioning and exercising
- Companionship and social support
Supported Independent Living (SIL)
- We offer 24/7 support with opportunities for capacity building to improve daily living and other skills while encouraging independence and ensuring your individual support needs are met
- We will support you with daily living tasks including grocery shopping, preparing meals, cleaning, personal care, medication management, appointments and any other daily tasks you may need
- We support you to manage money and household budgeting
- We promote participation in household and community activities of your choice
